1. Inside the Theorem Reach, scroll to the bottom of the page and click the question mark icon.

2. Choose 24 hours or last 30 days, then click on the survey that didn't credit.

3. Choose "I didn't receive my reward" from the drop down and fill out the form with as much detail as possible.
