1. Inside the Revenue Wall, scroll to the very bottom and click the "Contact Us" link.
2. Fill in the form and in the Message box explain:
- what the survey was about,
- how long it took you,
- the date and time you completed it,
- how much you think it was supposed to credit you, and
- why you think you completed it instead of just getting disqualified or screened out
